Daily Health Screening

Before entering school buildings each day, every student and staff member must complete the Health Screening Form to confirm that they are not experiencing any symptoms of COVID-19, have not recently tested positive, and are not currently required to quarantine based on close contact with an infected person. 

We recommend bookmarking www.healthscreening.schools.nyc on your device so you can quickly and easily complete the form before sending your child to school each day. 

Remember, if your child feels ill, do not send them to school! We want to keep your child and our school communities healthy and safe, and we must all do our part. 

Updated COVID-19 Testing Consent Form

Submit an Updated Testing Consent Form for Your Child in Grades 1st through 5th.

It is important that you provide consent for your child to participate in the in-school testing program. Submit your updated testing consent form for your child using your NYC Schools Account (NYCSA) at www.schoolsaccount.nyc.

Testing consent forms submitted in the previous school year expire on September 30, 2021. You must submit a new consent form for the current 2021-2022 school year. 

How to Consent

  • Submitting consent to have your child tested for COVID-19 in school is quick and easy. Please note-if you already submitted consent during the last school year or summer, you will need to do so again for the 2021-2022 school year. There are two easy ways to submit:

    • Fill out the form online using a https://www.schoolsaccount.nyc/

      • If you already have a NYCSA account linked to your student(s): Log in, click your student’s name, click “Manage Account,” and then when a dropdown menu appears, click “Consent Forms.” Read the page, and then choose the consent option at the end for your student.

      • If you do not already have a NYCSA account: You can create one right away! If you have an account creation code from your school, you can create a full account in approximately five minutes, and then provide consent as described above. If you do not have an account creation code from your school, you can still provide your consent right away by clicking “Manage Consent” under “COVID-19 Testing” and filling out your and your child’s information to provide consent.

COVID-19 Testing In Schools

Continuing the safety measures introduced in the previous school year, we will be conducting regular COVID testing of unvaccinated students grades 1 through 12. That way, action can be taken right away to protect the school community if there’s a positive case. In order for your child to participate in the in-school testing program, you must submit an updated testing consent form for your child to your school. You can fill out the form using your NYCSA account: https://www.schoolsaccount.nyc/ 

Visitor Policy

We enjoy visitors to Brighter Choice Community School. However, in order to proceed to the main office, room 103, the following protocol must be followed: 

Effective immediately, all visitors age 12 and older to DOE school buildings are required to provide proof of COVID-19 vaccination (1 dose), in order to enter the building, except in the case of an emergency. We ask that all visitors sign in with Security Agent, Ms. Brown with the following: 

  • A valid picture identification (ID), ​

  • Have proof of vaccination, ​

  • Complete the daily health screening form, temperature check and​

  • Wear a face covering.

Below are all acceptable proofs of vaccination: 

  • A CDC Vaccination Card, a photo or photocopy is also acceptable.  

  • A NYC Vaccination Record or other official immunization record from within or outside the US, including from a health care provider. A photo or photocopy of this card is also acceptable.  

  • NYC COVID Safe App on Android or iOS.  

  • DOE COVID-19 Vaccine Portal 

  • An acceptable COVID-19 vaccination includes one dose of any COVID-19 vaccine fully authorized or authorized for emergency use by the FDA, Food & Drug Administration, or WHO, the World Health Organization. 

BE ADVISEDNo strollers or shopping carts are allowed in or down the hallways of the school. There is stroller parking available at each entrance. If you are unsure where you may leave your stroller or cart, please ask the security agent for assistance.  

FOR THE SAFETY OF ALL SCHOOL STUDENTS AND PERSONEL any unauthorized persons on school property will be reported to the Principal, or their designee. Unauthorized or disorderly persons will be asked to leave and may receive a Limited Access Letter.  

Masks, Face Coverings, & PPE

All students and staff must wear a face covering when riding on school buses and anywhere on school property, indoors and outdoors, regardless of vaccination status, unless they have a medical exemption. Forgot a mask? No problem—every school will have a supply on hand for children and adults. 

During lunchtime, masks may be removed so students can eat comfortably at a safe distance from one another. Masks may also be removed during designated “mask breaks”, during which students will maintain physical distance from each other. 

Soap, hand sanitizer, paper towels will also be available in every classroom. 

School Cleaning

At BCCS we value our custodial engineers. They work hard to ensure that all classrooms and common areas, such as auditoriums and gyms will have hand sanitizer and disinfectant wipes available. High-touch” areas such as doorknobs and water fountains will be cleaned multiple times throughout the day, and each classroom and office area are deep cleaned and disinfected daily using electrostatic technology.
